I just wanted to vote for a superscripted (top?) link called "create" :)

As long as links are underlined this is no problem :). If you want to
give "the entire link phrase" as a reason you have to surround links to
existing pages too.
I'd suggest to mark links to other pages using a different way, perhaps
with a different background-color, a different border-line (dashed,
different colour?) or similar.
Another idea would be to use "+" or "+++" superscripted as textual icons
for "creation". I like them more than a ? which really looks like the
phrase stops there.
