How about
FIRST create a complete new pmwiki installation, and see that it
works, including creating new pages. And
SECOND upload all the page files fom the old wiki.d directory to the
new wiki.d directory. Also upload any directories and files in the
old uploads directory to the new upload directory.
THIRD customise the wiki with your favourite skin and custom scripts.
