If you use Fox you can add a name field to the form, and have the name
added to the notify=address line. This can be used to remind the admin
who subscribed, and also as a parameter for the notify email to add a
personalised heading.

To add to the simplest solution: You can just use a small mailform and
have the subscriber emailing you with his/her name, and you add them
manually to your subscriber list.
