Maybe a "lock this file" button on the edit page would do the trick?

By default, a file wouldn't get locked.  If a lock is necessary, click
the button.  The user might be able to choose the lock duration, subject
to constraints.

Such a button might be confusing for inexperienced users, but I'm
guessing that inexperienced users would just ignore it.  Besides;

   Simultaneous editing indicates a busy wiki.
   A busy wiki probably has experienced users.
   Experienced users could handle a lock-this-file button.

and

   Experienced users would discover the button's usefulness from an
    "experience", otherwise leave it alone (like HDD backups). :)
   If the button is ignored, the wiki would work as it does now.
   Presenting the button could be optional.

Maybe eventually there could be per-user / per-UserGroup / per-PageGroup
/ per-page settings for the lock-this-file button.  This would make
"prioritizing" possible.  Also, users who abuse their locking privileges
could have their maximum lock duration reduced.  Per-user / per-session 
tracking could allow someone to be kept from "hogging" a page (say, you
get two 2-minute locks per 10-minute period).

Just a some thoughts...

ps:  I keep making suggestions, but that's not to imply that PmWiki is
deficient in any way.  PmWiki doesn't *need* anything; it rocks as-is.
