My preference: [alt text] after images and abbreviations.

I'd used this as a local customisation for 
    [TIAA|This is an abbreviation] 
I agree that the alt text, acronym and abbreviation ought to use 
the same wiki markup. For some reason, I don't find 
    TIAA[This is an Abbreviation]
as natural as the [...|...] markup option, which is a bit silly,
since one naturally writes
    TIAA (This is an Abbreviation)

Agreed. I  don't like it for alt text either.

In all options, what happens with ' and " in the alt text? eg
a) if alt text contains ' and not ", alt="rock 'n' roll"
b) if alt text contains " and not ', alt='Play "Misty" for me'
c) if alt text contains ' and ", "You're fired!"

Or does it substitute the &quot; entity?
