Aha!  The Simple skin appears to define its own custom edit form in
$PageEditFmt, which completely overrides anything that might be
set or changed in Site.EditForm.  So, that's likely the problem.

(And yes, the Simple skin should probably be updated to use edit
forms in wiki pages instead of overwriting $PageEditFmt as it
currently does.)
