I think I have found an error in the syntax:

The above link shows a table that is supposed to have only three cells. 
However, on the last entry, there are four. I found the problem was with 
  markup within the table (specifically "[-(or the Black Hand)-]"). If I 
removed the aforementioned markup, the table rendered as expected.
