1) The problem is that, without an "unblock" procedure I end up having
to maintain far too many blocklists. If I have 100 fields in my farm
and I want all but one to block most medical terms, that means
maintaining a blocklist for the medical terms in 99 different fields
since I can't put the med terms in the sitewide blocklist.

Or am I misunderstanding something?

2) Blocking URL fragments is fine-- the problem is that by the time I
know of the new fragment-- since there is no sharing of blocklists-- I
already have hundreds or thousands of pages of spam to deal with!

3) Words may be hard to define, but the vast majority of spam on my
wikis comes in the form of whole words that I'd like to be able to
block. Being able to block "tit" and "cock" as whole words would save
me a ton of time. Similarly, I'm amazed how many of the medical terms
come up as subterms in other situations, both in words and
particularly in valid URLs

The spam issue has become severe enough that if I don't have the
blocklist my users are abandoning the use of wikis. And if I DO have
the blocklist, the current incarnation interferes with users to the
point that they also get frustrated. Granted, many of my users are new
to the idea of wikis and it's my job to help them take the risk and
get the results they are looking for-- and it doesn't take much to
scare them off :)
