By popular request, I have uploaded the recipe for 'PITS on steroids' that:

- uses a wiki page as a form template

- lets authors edit existing pages using a form, as well as
  create new pages

- supports a range of form elements including pick lists,
  radio buttons, check boxes and text areas

- can apply a range of wiki markups, including [[~author]],
  [[!category]] and [[name -> mailto:address]]

- lets an administrator specify which elements to include
  in a page list, and how to sort the list, via the template

- one template page per group, or put all template pages in
  a forms group

See http://www.pmwiki.org/wiki/Cookbook/WikiForms

An example is at http://intranet.affinity.co.nz/pmwiki2/index.php?n=Issues.Issues

It's a beta!! In particular, the markup is OK, but not quite right.

I know of at least one bug.

Questions, suggestions and comments welcome.
