okay I found a bug with page header. It is now fixed. Sorry it took so
long till I got to run some tests!

The skin will now always use Site.PageHeader (or a Group.PageHeader if
it exist), and a  default Site.PageHeader is loaded from the skin's
wikilib.d directory.

If $PageLogoUrl is defined it will be used (otherwise the
default pmwiki logo appears).

Site.PageHeader can also be edited to be customised as needed.
The customised version will then override the skin's default version
of the page.

The same bug was also in Gemini skin. Both FixFlow and Gemini are
updated.
