I'm sorry, I missed the discussion and don't know if and what is the
result by now, but was

already suggested? This might give problems though with...

having four brackets and nesting tags...

A cute figure: [[HappyFace [[http://foo.bar/image.gif happy face]]]]

What about leaving it as it is and putting the image name into the alt
tag? This way one would make sure that everywhere the same alt text is
used and that the image gets a nice filename (having some "_" in it
though. 

BTW - if you start with alt text then keep in mind that another W3C
recommendation is to use the title-attribute for links, images, etc.
