I have custom formatted by RecentChanges file so it shows time, author and
IP.
In config.php - below include(cookbook/extendmarkup.php) if you use it

################## this needs to be loaded after Markupextension FIX
###################
     $RecentChangesFmt['$Group.RecentChanges'] =
        '* [[$FullName | $Title]]  . . . $CurrentTime $[by] $AuthorLink
($LastModifiedHost)'.        '$ChangeSummary';
################## END Markupextension FIX ###################

The IP address shows up and you can add it - or the block - to the
blocklist2 page
