You appear to be using a variation of the Light skin
(http://www.pmwiki.org/wiki/Cookbook/LightSkin).  That
skin uses a different page for its edit form -- Site.LightEditForm.
Try making modifications to that page instead of the PmWiki default
Site.EditForm.
